Medication, if necessary and agreed upon, will be called in to the patients preferred pharmacy and any remaining questions or concerns will be addressed. A follow-up visit to assess ongoing effectiveness of the treatment plan will be made. At consecutive appointments, any necessary changes to the treatment plan will be made. However, I am available for concerns, questions, or needs that may arise prior to follow-up visits when needed.
